Parent and Guardian Consent Form

This form is for the parent or legal guardian of every student under 18 who joins Sikhi Noor Academy's live online classes. Adults enrolling themselves sign it for themselves. It takes about two minutes to read.

1. Who I am

I am the parent or legal guardian of each student I enrol, or I am an adult enrolling myself. I have the authority to agree to this form for them, and the contact details I give are correct.

2. Joining classes

  1. Classes are live on Zoom. I will keep each class link private and will not share it or post it anywhere.
  2. I will make sure my child joins from a quiet place and has a working device and connection. For younger children, an adult will be nearby and available during class.
  3. I will ask my child to be respectful to teachers, other students and the teachings, as set out in the Terms. The academy may remove a student from a class, or pause an enrolment, if behaviour disrupts others.

3. Recording

A class may be recorded so enrolled students can rewatch it and so we can keep teaching quality high. Recordings are private, are shared only with families in that class, and are never posted publicly. The full rules are in our Recording and Child Safety Policy.

4. Photos and video (optional, separate)

Photos or short clips of my child in class may be used on the academy's website and social media only if I tick the separate optional box at checkout. If I do, they will not be shown with my child's full name or contact details. Leaving that box blank does not affect the enrolment. I can withdraw this choice at any time by emailing the address below, and future posts will stop using my child's image.

5. Contact and personal information

The academy may contact me by email and WhatsApp about enrolment, class times, class links, progress and important updates. My information is handled as described in the Privacy Policy. Payments, refunds and cancellations follow the Terms and the Refund Policy.

6. Changing my mind

I can withdraw any part of this consent at any time by emailing hello@sikhinooracademy.com. Withdrawing consent to the core parts of this form means my child can no longer attend classes.

How you sign. At checkout you tick the consent box and type your full name. That counts as your signature. We keep a record of your name, the date and time, the version of this form, and your photo and video choice.
